Referral Information

REFERRING TO REFLECTIVE PRACTICES & SUPERVISION

Reflective Practices & Supervision accepts referrals from various avenues. My referrals are from General Practitioners, Psychiatrists & Medical Clinics.

My most common referrals are GP Mental Health Care Plans and Private/Community Referrals.

Reflective Practices & Supervision does not offer Bulk-Billed Sessions.

HOW DO I REFER TO Reflective Practices & Supervision?

When completing your GPMHCP referral, I recommend utilising my checklist below to ensure I receive all the information I require. Please send any additional documentation available (e.g., GPMHCP or GP referral letter).

Referrals can be sent via email or fax. I also use Medical Objects.

WHAT HAPPENS ONCE I HAVE REFERRED MY CLIENT TO YOUR SERVICES?

Once I receive a referral, I will make contact with the client and book in their initial session. Once the client has attended their first session, I will write a letter to the referrer. I uphold a professional relationship with referrers and will always make contact if there are any concerns.

General Practitioner (GP) Referrals

Reflective Practices & Supervision welcomes GPMHCP referrals. If you are wondering if Reflective Practices & Supervision is the right service for your client, please read more about my service and the specific areas of therapy I provide here. If you would like to further discuss a potential referral, please don’t hesitate to contact me via email at admin@reflectivepractices.com.au or,  on (07) 5240 9006 , Tuesday - Thursday during business hours. I will always respond to you from Monday to Thursday (excluding public holidays) within 24 hours of your email or voice message.

DOES THE REFERRAL MEET MEDICARE REQUIREMENTS?

Reflective Practices & Supervision requires all GPMHCP referrals to meet the Medicare requirements. To make it easier for you, I have provided a checklist of essential details below.

Your referral MUST include:

 Patient’s name

 Patient’s date of birth

 Patient's residential address

 Patient’s contact phone number

 A list of the patient’s current medications

 An eligible diagnosis

   The number of sessions the patient is being referred for:

*6 Initial Sessions under a MBS Mental Health Care Plan       

*4 Additional sessions under a MBS Mental Health Care Plan (to make up the initial 10)

 Referring GP name

 Referring GP practice

 Referring GP provider number

 Referring GP signature

* This information can be in the form of a signed and dated referral letter.

Other Clinic Referrals:

Reflective Practices & Supervision welcomes referrals from members of the community wanting to refer themselves or from other professions wanting to refer people to Reflective Practices & Supervision.
